Chrissy - Assistant
Chrissy is from Groveport, Ohio. She has been assisting since August 2015, and is certified in radiography, as a phlebotomy technician, and in first aid. Chrissy has also received certifications in infection control and chairside assisting through the Dental Assisting National Board (DANB). Chrissy’s goal is to make every patient feel at home when at Fulks Family Dental by creating a stress free environment for them. She also understands that some patients experience anxiety about coming to the dentist and provides extra care to ensure that they feel comfortable during their visit and informed about their care. In her spare time she enjoys spending time outdoors with her family and her furbaby, Brady. Chrissy and her fiancé, Jonathan, are always on the lookout for a good concert to attend.
Leah - Hygienist
Leah was raised in Marysville, Ohio. As the dental hygienist for Fulks Family Dental she works with patients to reduce the risk of tooth decay and gum disease. Leah graduated from the University of Cincinnati, in 2010, with her degree in Dental Hygiene. She is certified to administer local anesthesia and nitrous oxide. Leah joined our office in April 2017. She enjoys working with patients to improve their oral health and strives to provide a gentle and positive dental visit. When Leah is not working she enjoys traveling and exercising, as well as raising her two boys with her husband, John.
Samantha - Assistant
Sam is from Columbus, Ohio. She has been a dental assistant since 2012 after she attended Fort Hayes High School Dental Assisting Career Center. She holds certifications for the Dental Assisting National Board (DANB), Phlebotomy (CPT), Coronal Polishing, Nitrous Oxide monitoring, CPR, and first aid. Sam joined Dr. Fulks and his surgery team in May 2016 and then joined the Fulks Family Dental team in January 2017. Sam’s greatest pleasure is getting to know her patients and making their dental experience comfortable and anxiety free. In her free time Sam enjoys attending country concerts with her family and spending time with her two nephews and niece.
Dr. Rebecca Bartlett
Dr. Rebecca Bartlett grew up in Youngstown, Ohio and graduated valedictorian from Boardman High School. She then earned a Bachelor’s degree in Biochemistry graduating magna cum laude from John Carroll University, and went on to earn her Doctor of Dental Surgery degree from The Ohio State University College of Dentistry. Following dental school, Dr. Bartlett completed advanced training in Oral & Maxillofacial surgery and IV sedation from the University of Kentucky as part of one-year internship. She is comfortable treating complex patients and has experience placing implants as well as performing a wide variety of dentoalveolar surgical procedures and administering IV conscious sedation.
Dr. Bartlett loves to give back to the community and has been involved in international service trips since college, including traveling to both Jamaica and Haiti to serve others. In dental school, she was heavily involved in organized dentistry through the American Student Dental Association, and was President of the Ohio State’s Give Kids a Smile program that provides complimentary dental services to children in need. She is currently a member of both the American Dental Association and Ohio Dental Association, and hopes to increase participation in organized dentistry as her career progresses. Dr. Bartlett is excited to join the Fulks Family Dental community and provide the utmost quality dental care to her patients. In her free time, she loves to stay active and be outside as much as possible biking, rock climbing or hiking and enjoying the variety of food and activities Columbus has to offer.
Dr. David Fulks
Dr David Fulks was born and raised in Athens OH, and graduated from Athens High School in 2001. He earned his undergraduate degree in Biochemistry from Virginia Tech in 2005 and his Doctor of Dental Surgery (DDS) degree from The Ohio State University College of Dentistry in 2009. He attended Stokes VA Medical Center General Practice Residency Program from 2009-2010 where he received his GPR certificate and extensive training in oral surgery and anesthesia. In 2012, he received his IV Conscious Sedation certificate from Miami Valley Hospital in Dayton. Dr Fulks is a member of The American Dental Association, OH Dental Association, NW OH Dental Association, Academy of General Dentistry, American Academy of Implant Dentistry and American Dental Society of Anesthesiology.
Dr. Cody Miller
Dr. Cody Miller was born and raised in Lima, OH and graduated from Bath High School in 2006. He earned his undergraduate degree in Applied Health Science from Bowling Green State University in 2010 and received his Doctor of Dental Surgery degree from The Ohio State University College of Dentistry in 2016. He received his IV sedation certificate from Miami Valley Hospital in 2017. Dr. Miller is an active member of the American Dental Association, Ohio Dental Association, and Academy of General Dentistry. In his free time, he enjoys being outdoors hunting and fishing and spending time with his family.
Dr. Nicolas Eberly
Dr. Eberly grew up on the East side of Columbus, attending St. Catharine’s Elementary School and St. Charles College Preparatory High School. He went on to attend Miami University, graduating cum laude with degrees in Zoology and Neuroscience. He then attended Ohio University Heritage College of Osteopathic Medicine in Athens, OH, where he served as the Surgery Club President and would eventually match into his top choice residency program.
Currently, Dr. Eberly is an Otolaryngology (Ear, Nose, & Throat Surgery) Resident Physician based out of Columbus, Ohio. He has spent years studying complex facial anatomy and learning both the surgical and non-surgical treatments for diseases of the head and neck. His passion and interest for Facial Plastics and Reconstructive Surgery has inspired him to dedicate his future to this highly subspecialized field, with plans of pursuing a prestigious fellowship position in American Academy of Facial Plastic and Reconstructive Surgery.
